determine residents’ level of Internet connectivity in their respective
communities. The survey and speed test can be accessed via

Residents can also complete a paper survey, by calling *1-855-NYBBMAP*
(1-855-692-2627)
and following instructions to have a paper copy mailed directly to their
home.
